FYI: Eotech was recently sued by the US government based off a quality issue against the EOTECH Holographic sight system. In short the optic can float out of accuracy if exposed to extreme temperatures.
The rebate is simply asking for the serial number, model, and date of manufacture. How the cost is determined or validated is unclear to me at this point. I revisited the original web site and looked it up.
