My starboard front grab rail is really loose but I have no idea how to get at it to tighten it. Does anyone know how to remove the plastic insert with the cup holder and courtesy light?

Looks like it may just snap in here is Hull Fittings for your boat it may help you visualize the installation.
 
I just tightened mine on a 242X. The panel has four small studs that I had to remove the small nuts off of and remove the cup holder before I could remove the small panel. Was easy to tighten after the panel was removed.
 
@Sean Clements beat me to it, but he is exactly right - our 212x are same.

I'm having the same problem. How exactly did you get the insert off? I've tried pulling fairly hard to see if it snaps out but I feel like I'm gonna break it
 
Mine were loose also. I laid inside the compartment facing up used a ratchet with a long extension and a swivel universal. Worked just fine
 
13 mm deep well socket and my son that fits in the compartment laying down
